By enrolling my Camper in the Restoration Fellowship FireKids Ministry VBS Camp 2025:
I agree to pay the Camp fees no later than (June 6th).
I understand that a reserved space may not be held without full payment. REFUND/CANCELATION POLICY: A full refund is available if canceled 7 days before the start of Camp and partial refund if canceled 48 hours before the start. There will be no refunds if canceled after this time period. Extenuating factors may be considered.
I grant permission for my Camper to participate in all planned Camp activities.
I grant the Restoration Fellowship Fire Kids Ministry and its volunteers full authority to take whatever actions they deem necessary regarding my Camper’s health and safety, and I fully release Restoration Fellowship Fire Kids Ministry from any liability in connection there within. In the event of an emergency, I understand that prudent attempts will be made to contact the undersigned immediately.
I understand that my Camper must comply with the Camp’s rules and standards of conduct and that if deemed necessary, Restoration Fellowship Fire Kids Ministry may terminate my Camper’s participation in the Camp program if he/she does not maintain these standards.
I understand that Restoration Fellowship Fire Kids Ministry is not responsible for lost, stolen, or damaged personal articles.
I authorize Restoration Fellowship Fire Kids Ministry to own and have full right of disposition to any video film, footage, sound recordings, and photo reproductions of the Camper named on this form.
I acknowledge that with participation carries risk of physical injury.
I agree that Restoration Fellowship Fire Kids Ministry, and its volunteers shall not be liable for any injury or damage, howsoever caused, resulting directly or indirectly from the Camper’s participation in Restoration Fellowship Fire Kids Ministry VBS Camp 2025.
I consent to the collection and use of my child’s personal information by Restoration Fellowship Fire Kids Ministry for the purposes of camp enrollment, safety, communication, and participation in camp activities.
I understand that this information will be securely stored and only shared with authorized staff and service providers as necessary to ensure my child’s well-being and the effective operation of the camp.
